About Substrate Labs

Substrate Labs is a computational life sciences company based in Mumbai, India. Substrate Console, its first product, keeps a clinic's queue, consultation notes, prescriptions, documents, and lab results in one patient record.

Substrate Console is the record for a working clinic. Today's queue, consultation notes, prescriptions, documents, and lab results, kept under one patient and signed by the doctor who wrote them.

    Add the patient to today's queue.

    Write a structured note with vitals and diagnosis.

    Build the prescription inside the same visit.

  4. Sign

    Sign, print, and file it to the patient's history.

Drafts save themselves

Close the tab or lose the connection. The note is where you left it.

Signed, then locked

A signed note cannot be edited in place. It carries your name and the time.

Amendments leave a trail

Every change after signing needs a reason and keeps the earlier version.

Prescribe by brand or generic, from the National List of Essential Medicines and your clinic's own brands. Instructions in English, Hindi, or Marathi. Printed on your letterhead with your signature.

Second look.

Before you sign, the prescription is checked against what is already on file. These are rule-based warnings, not diagnostic or treatment recommendations. The decision stays with the doctor.

Every consultation, prescription, document, and lab result, in one timeline. Filter by type or date, and open the original from any item.

Documents in, values out, duplicates caught. And your old records come with you.

Upload a PDF, an image, or a photo from your phone. Tag its type and date.

Lab results

Enter values once. Anything outside the entered range is flagged, and trends are charted.

Search by name, phone, patient number, or ABHA address.

Duplicates, caught

Registering someone who may already exist shows the match. Merge the records, or unmerge them later.

Bring your old records

Import patients from a CSV with a preview and row-by-row errors. Upload documents in bulk. Running it twice will not create duplicates.

ABHA, at hand.

Patients scan a code at the desk and check in with their ABHA app. They are matched to their existing record and added to the queue. Signed records are shared only with their consent.

ABHA features are currently available in test mode and will go live after ABDM certification.

HFR and HPR IDs

Your clinic's facility ID and each doctor's professional ID are stored on their profiles.

Scan & Share check-in

A scanned patient is matched to their existing record and added to today's queue.

Consent stays with the patient

Records are shared as FHIR care contexts only after the patient consents. Every share is audited.

Nothing off the record.

Everyone sees what their role needs. Every sign-in, signature, view, and share is written to the audit log.

Admin, Doctor, Staff

Invite people by email, change their role, or deactivate them. One account can run more than one clinic.

Audit log

Sign-ins, record changes, signatures, document views, shares, and imports. Filter by person, action, or date.

Quiet email

Invitations and administrative notices only. No clinical information is ever sent by email.
